If you would like to set up a time to come in for counseling, please fill out form one and form two. The first is a Counseling Application: Personal Information & Agreement form. This form gives us basic information about you so that we can contact you more easily in case something comes up that might affect our time together. It also asks you to read, understand, and agree with various aspects of what we do at the Center for Biblical Counseling.
The other form is a Personal Data Inventory. It asks you to give more personal and background information as well as a short description of why you want to visit with us and what you would like us to help you with. It helps us to get to know you and your situation a little bit before you get here.
After we receive and review the forms, someone from our office will get back to you to schedule an appointment.

Things to know and do if possible:
It is helpful to know that the meetings will last 50 minutes or less. The first visit might be longer depending on the information being gathered.
You could get a huge jump on counseling if you would observe this video and do what Dr. Lawyer suggests before your first meeting.
Also, please bring a Bible and something to take notes on to all counseling meetings.
